Enrollment Guidelines & Document Requirements

Initial Requirements for Qualification
To begin your application, please prepare the following:
PHP 500 Qualifying Fee
Photocopy of your PSA Birth Certificate
Photocopy of your School Report Card
Two (2) pieces of 2x2 ID photos (with your name written on the back)

Documents to Submit Upon Qualification
PSA Birth Certificate
Form 137 and Form 138 (for Senior High School graduates)
High School Diploma
Transcript of Records (for college graduates)
College Diploma (if applicable)
Valid Passport
Updated Resume
Medical Certificate (can be submitted later if not yet available)
